REGISTRATION FORM FOR THE WORKSHOP

The form shoud be filled and sent back to ESAU secretariat on esauganda@yahoo.com or info@epilepsy org.ug before 15th November 2008. Participants seeking sponsorship from Epilepsy Support Association Uganda (ESAU) should fill the form giving reasons why they want to be sponsored. They should support their reasons by indicating how they have helped the lives of people with epilepsy easier in their communities.
